Thank you for your request under the Official Information Act 1982 (the Act) on 14 July 2021 for 
several  COVID-19  documents.  You  stated  you are  interested  in “…copies  of  three  documents 
mentioned in the COVID-19 Independent Continuous Review, Improvement and Advice Group’s 
review of the February outbreak response.” 
Please find a response to each part of your request below. 
“1) Auckland February Outbreak 2021 – Internal Review of the Auckland February 
Outbreak 2021 Response 
2) Auckland February Outbreak 2021 – Internal Review of the Incident Management
Team Response”
While the Ministry of Health (the Ministry) has endeavoured to search for any information within 
the  scope  of  this  part  of  your  request,  only  one  internal  review  document  was  found.  This 
document  is  outlined  in Appendix  1  with  a  copy enclosed  with  this  letter.  No  other  documents 
were found within the scope of the above request. Therefore, part of your request for information 
relating to the “Internal Review of the Incident Management Team Response” is refused under 
section 18(e) of the Act, as the information requested does not exist, or despite reasonable efforts, 
cannot be found. 
“I'd like this to include the two collateral documents mentioned on pages 17-18 of the 
report which are described as the “February Cluster Debrief” and the “brief memo on 
media information leaks.” 
The document titled “February Cluster Debrief” has been identified and outlined in Appendix 1 
with a copy enclosed. The document titled “brief memo on media information leaks” is outlined 
in Appendix 1 but is withheld in full under section 9(2)(ba)(i) of the Act, to protect information 
that is subject to an obligation of confidence and making it available would likely prejudice the 
supply of similar information, or information from the same source, and section 9(2)(ba)(ii) of the 
Act, to protect information that is subject to an obligation of confidence and making it available 
would likely damage the public interest. 
Please note, the Department of Internal Affairs did not receive a copy of the “February Cluster 
Debrief” document and therefore had no opportunity to comment.

This document will soon be proactively released on the Ministry website. Therefore, this part of 
your request is refused under section 18(d) as information requested will soon be made publicly 
available.
