We  refer  to  your  request  dated  2  February  2014  and  received  in  this  office  on  
3 February 2014 under the Official Information Act 1982 (OIA) requesting information as 
follows. 
 
1.  The total number of hemipelvectomies performed by the CDHB over the last 
5 year 
(i) the total cost of those surgeries; and 
(ii) the total rehabilitation costs to date associated with those surgeries 
 
2.  The total number of pelvic restoration surgeries performed by the CDHB over 
the  last  5  years  (for  example  due  to  osteosarcoma,  motor  vehicle  accident 
trauma and the like) where those surgeries used custom implants 
 
(i) the total cost of those surgeries; and 
 
(ii) the total cost of the custom implants used in those surgeries 
 
3.   The total number of: 
 
(i)  spinal fusions; and 
 
(ii) spinal fusions using anterior lumbar inter-body fusion (ALIF) devices for 
each of the last 5 years by the CDHB; and 
 
(iii) the total cost of those surgeries for each year and (iv) the total cost of 
the ALIF devices for each year
