Exterior Examination;
An exterior examination of the property and surrounding area, was conducted.
I found no items which could not be accounted for and/or suggested they
were/could have been used as an accelerant or ignition source for the fire.
Fire had severely damage the right end structure (looking from the front), more
precisely the roof area which had dropped down, but still intact, due to possible
burning of the wooden framing inside.
Discolouring of the iron on the roof had also occurred in this area.
The left side of the roof appeared to be less damaged.

A heat pattern (blacken iron) was noticeable externally at the right front corner of
the garage.
A “V” heat pattern was noted on the iron cladding near the right roller door.
Heat and smoke damage was noted to the right gable end of the garage.
Windows in the shed were broken, although it was not possible to confirm if this
had occurred prior to the fire, or occurred due to the fire, or from fire
extinguishment by FENZ
The single door at the right end of the garage was missing.
The two single iron roller doors near the centre and left of the garage had been cut
open by FENZ.
Due to fences and vegetation it was not possible to do a complete 360* of the
outside.
Interior Examination;
I entered through the left roller door, where there was an old-style vehicle parked
Act
with a cover on it.
Heat and fire appears to have caused some damaged to this vehicle
There was shelving, work benches and various cabinets against all the walls.
A pool table was near the single door at the right end, and the compressor was
near the single door beside (in front) of a fridge.
Next to the single door, half way up the wall, was a small electrical meter box,
which appeared to have melted during the fire, rather than some possible form of
electrical malfunction occurring.
An examination of the extension cord from the compressor and both electrical
power plugs which both items were plugged into, were examined and there
appeared to be no sign of any malfunction.
The timber roof framing at the right end (approximately above the pool table) had
Information
burnt away, causing the roof to collapse in that area.
Burning patterns on the wall timber framing and items in this area suggests fire has
come down onto items below, rather than from low down up.
Above the pool table on the roof framing, was the electrical wiring from four lights,
one light was hanging down but still intact.
Area of Origin;
In my opinion the area of origin was in the electrical wiring and/or light fitting at the
Official
right end of the garage.
Point of Origin;
The point of origin was undetermined, with the most likely being some form of an
electrical malfunction in the electrical wiring and/or light fittings, above the pool
the
table.
This has resulted in an electrical event from which arcing, sparking and possibly
burning material has emanated.
This material has ignited other flammable items nearby (most possibly on
flammable items stored below) and fire has then spread throughout the entire
structure.
under
In my opinion this fire had been burning for some time, before being reported.
CONCLUSION;
Based on evidence and witness statements available at the time of this
investigation, in my opinion the classification of this incident has been recorded as
Accidental.
OTHER CAUSES CONSIDERED;
An unlawful fire cause was considered and was eliminated due to insufficient
evidence or witness statements.
Natural fire causes were eliminated as there was no evidence in the areas of origin
of chemicals prone to spontaneous reactions or natural weather phenomena
Released
having taken place.
A discarded cigarette and/or match was considered and eliminated, due to
insufficient evidence or witness statements.

F2960283 – 134 Colombo Rd Waiuku;
On Tuesday 3rd March 2020 -- I was the on-duty Fire Investigator, and was notified
at 0755hrs to attend a structure fire at 134 Colombo Rd Waiuku;
This call had been received by FENZ comms, via the 111 system on the 5
Act
February 2020 at 0732hrs.
FRMO
 (Trainee) attended this incident with me.
The reason for FRMO
 attendance was to assist and learn the skills
required to confirm the origin and cause of the fire.
We arrived at the incident at approximately 0845hrs.
The reason for my attendance was to assist the Fire and Emergency Officers to
confirm the origin and cause of the fire.
Upon our arrival, the OIC (SO
) gave a brief of the scene at the time of
FENZ arrival, and actions taken in fire extinguishment.
The OIC’s and fire crew’s information and evidence informs this report.
The weather was calm and there was no wind at the time of my inspection.
The owner/s of this property,
 was present at the time of my
Information
investigation, and gave permission to investigate the origin and cause.
I understood members of the household did smoke, but were not in the vicinity, or
at the time of this incident.
This incident was in a wood framed, iron clad, double garage approximately 9m x
5m with a concrete floor.
There was electricity to the garage.
 had used the compressor in the garage at approximately 0545hrs to pump
up
 car tyres, but can’t remember if
 turned it off. Official
 advised there had been rodent issues in the past, although there had been
no electrical concerns (faults/malfunctions) in the garage.

 who was present at the time of our inspection, indicated
 had
heard a loud bang just prior to the fire.
the

 evidence and information informs this report.
Amongst the items inside the garage were a classic car and Harley Davidson motor
bike, pool table, ride on mower and many tools and equipment common with
mechanical and motor vehicle repairs and maintenance.
Containers of paints and fuel/oils were also present, some in origin of this fire.
With evidence, available at the time of my investigation and items like paints and
under
fuels in the garage I decided not to conduct a PID test.
A ppbRAE 3000 photoionisation detector (PID) can be used to test for the
presence of volatile organic compounds (VOCs). The presence of VOCs may
suggest a flammable liquid has been present at the time of a fire.
