Information Request - Operational Reviews - Vegetation Fires 
I refer to your official information request dated 6 December 2019 asking for the following 
information: 
1. A copy of the Operational Review undertaken on the Flagstaff Walk Track Vegetation Fire -
F2844357.
2. A copy of the Operational Review of the Lake Hawea Vegetation Fire - F2642681.
3. Copies of any other Operational Reviews undertaken on Vegetations Fires over the last 2
years.
Fire and Emergency New Zealand is committed to continuous improvement.  There are always 
lessons we can learn from large or unusual events to improve the safety of our communities and 
our crews at future events. The Operational Review process provides an effective way for us to do 
this. 
In accordance with the provisions of the Official Information Act 1982, I enclose copies of the three 
Operational Reviews carried out on vegetation fires over the last two years at Himatangi Beach, 
Lake Hawea and Dunedin. 
The Operational Review reports for Port Hills and Tasman fires are available on our website

You have the right to seek an investigation and review by the Ombudsman of this decision.
