We are in receipt of your request under the Official Information Act (1982), dated 5 August 2015 and 
subsequently  transferred  to  us  from  the  Ministry  of  Health  yesterday  requesting  the  following 
information.  
 
  All contact details for the internal numbers and external numbers for calling all hospitals and 
medical clinics under the Christchurch District Health Board.  This is to provide patient care to a 
social cause that is embargoed. 
 
As  required  under  the  Official  Information  Act  (1982),  the  Canterbury  District  Health  Board  will 
respond to your request within 20 working days from receipt of your request. You should therefore 
receive a response on or before 8 September 2015. 
 
If a large amount of information has been requested or if the Canterbury District Health Board needs 
to  consult  in  order  to  make  a  decision,  we  may  need  to  extend  this  date  (this  is  provided  for  in 
section 15A of the Act).  We will advise you if such an extension is necessary.
