Thank you for your email of 25 March 2018, to WorkSafe New Zealand (WorkSafe)
requesting the following information:

Could I please request all WorkSafe reports and investigation information for
deaths that have occurred at zoos and as a result of accidents that have occurred
at zoos.
Your request has been considered under the Official Information Act 1982 (OIA).
Firstly, WorkSafe publishes a summary of fatalities on our website that you may wish to
refer to: www. worksafe. govt. nz/data-a nd-resea rch/ws-data/fatal ities/workplace-
fatal ities-su m ma ry/. I would encourage you to explore the information that is publically
available.
I have interpreted your request to be for all WorkSafe (or its predecessor the
Department of Labour) investigation reports for fatalities that have occurred at zoos for
the period 2012 to 25 March 2018.
Therefore, please find attached two investigation reports considered to be within the
scope of your request. Some information has been withheld under section 9(2)(a) of the
OIA, to protect the privacy of natural persons, including that of deceased natural
persons.
As per section 9(1) of the OIA, I do not consider that, in the circumstances of this case,
the withholding of the information referred to above is outweighed by other
considerations which render it desirable in the public interest to make the information
available.
You have the right, by way of complaint under section 28(3) of the OIA, to request an
investigation and review of this response by the Ombudsman. Information about how to
