Decisionmaking on passport application form

Dear Department of Internal Affairs,

1. I request information on the way that decisions are made on the form(s) used by people to apply for passports.

2. I am happy to amend this request if that facilitates your response. Please contact me in this case.

3. Section 4(2) of the Passports Act says:
"4 Issue of passport
(2) Every such application shall be made to the Minister in the form provided by the Secretary."

3. You have online three forms - adult renewal, adult new, and child.

4. I make this request either in respect of:
- the form which has been changed in the last five years, and which has been considered by the highest level of decisionmaker (if this varies)
- or if the most recent change was over five years ago, the most recently changed form

5. I request in respect of this one form:
- the date of the form (month/year)
- the decisionmaker - Secretary of Internal Affairs or other
- whether the change(s) in the form were referred to the Minister of Internal Affairs, whether for noting or approval
- whether the change(s) in the form were referred to other Minister(s), whether for noting or approval, and if so which Ministers
- whether the changes in the form were the subject of a Cabinet committee paper, whether for noting or discussion or approval
- whether the changes in the form were the subject of a Cabinet paper, whether for noting or discussion or approval
- a copy of the highest level paper produced. That is, Cabinet paper and minute (if any), or Cabinet committee paper and minute (if any), or document signed off by the Secretary of Internal Affairs, or other document.
