Under the OIA, I request a copy of the following: 
 
The Risk assessment that was used to class Mental Health & Addiction Services as High Risk 
in the internal Visitors and Nominated Support Persons – Standards Policy 6.9.4 Protocol 1 
(1.1 - 1.2). 
 
Response 
There was no specific risk assessment document utilised to class Mental Health and Addiction 
services as High Risk in this instance.  This assessment was made by senior staf  based on 
the clinical and operational knowledge of the service.
