Thank you for your request under the Official Information Act 1982 (the Act) to the Ministry of 
Health (the Ministry) on 14 January 2021 for: 
“.... I would like to request a copy of your current organisation chart covering the top three 
tiers (tiers one to three). If at all possible I would like the names of the managers as well 
as their titles - if that is not able to be provided I would like to understand why.” 
Information in response to your request is as follows.  
Tier One is Dr Ashley Bloomfield, Director-General of Health and Chief Executive. 
Tier Two Ministry staff are the Executive Leadership Team which is publicly available:

Tier Three Ministry staff report directly to the Executive Leadership Team. Please refer to 
Appendix 1 enclosed which was extracted on 19 February 2021. Senior Executive Assistants 
and job position numbers have been excluded as outside the scope of your request. Please 
note, this is a dynamic document which is updated daily in accordance with recruitment 
changes.
